ATTENTION!

Starting February 18, 2008 applications for scholarship grant will be accepted. (Last day of filing will be on March 31, 2008.)

1. For current scholars who will renew their applications, please bring the following requirements:
a. Letter of intent addressed to NSDAPS Director
b. Clearance from the school Cashier OR statement of outstanding balance
c. Income Tax Return (ITR) OR Certification of Exemption to file ITR (for parents of Director's scholars only)
d. Photocopy of Report Card (with at least the 2nd periodic grades)
e. 1 x 1 ID picture

2. For NSDAPS Employees' scholarship grantee, please submit letters a, b & e requirements.

3. New applicants who are presently enrolled at NSDAPS will have the same requirements a to e.

4. New applicants who are from other schools will have the ff. requirements:
a. Letter of intent addressed to the School Director of NSDAPS
b. Certification of honor from their School Principal
c. Income Tax Return (ITR) or Certification of Exemption to file ITR
d. Photocopy of Report Card (with at least the 2nd periodic grades)
e. 1 x 1 ID picture

ONLY APPLICANTS with complete requirements will be issued an application form at the Registrar's office.
